Do you need an assessment to help you apply for NDIS funding?

We can offer Preliminary Functional  Assessment service at our Elizabeth and City West clinics for people between 18-65 with neurological conditions such as stroke, Parkinson’s disease, and acquired or traumatic brain injury.

Assessments take between 90 and 120 minutes and cost $250.

Under the supervision of a qualified Occupational Therapist, Occupational Therapy students will assess how your neurological condition impacts your day-to-day function and compile a full report that you can use as evidence to support your application.

These are not a full functional capacity assessments but can be used as evidence of your eligibility for a functional capacity assessment to be funded by the NDIS.

What is the difference between a full Functional Capacity Assessment (FCA) and our NDIS Access report?

Our NDIS access reports are designed to help clients gain access to NDIS support. They serve as valuable evidence, much like the NDIS Access Request Form required for NDIS applications. The reports focus on demonstrating the client’s level of disability and their need for support in key areas like mobility, communication, social interaction, and self-care, as required by the NDIS Access Request Form.

A Functional Capacity Assessment is an evaluation used to determine a person’s functional abilities and limitations in performing everyday activities across all domains. It provides detailed insights into the person’s physical, cognitive, and emotional capacities, often with specific recommendations tailored to their needs and goals.

 

Who will be conducting the assessments?
Students will conduct the assessments under the supervision of a licensed Occupational Therapist. Both the students and the Occupational Therapist will sign off the report.

 

How long does the assessment take, and what is the fee?
The assessment typically takes 1-2 hours and costs $250 in total. Students will engage with clients through questioning and conduct assessments if required. Comparatively, for individuals receiving NDIS funding, an OT full FCA report usually costs around $2k ($193.99 per hour x 10 hours).

Unfortunately, the $250 fee cannot be claimed through private health insurance as this service is student-led.

 

When do clients pay?
Payment is due after the interview. Students will guide clients to reception to ensure payment is made. The report will only be released once the fee is paid.

 

What do clients need to do after confirming the appointment?
Upon confirmation, the admin will send an email with the appointment details and invite the client to complete attached assessments recommended by the supervisor. These assessments include WHODAS, WHOQoL, PDQ39, and MoCA.

The assessment will be collected at the face-to-face appointment. If clients do not have emails or have not returned the assessment, they can complete it onsite during the face-to-face meeting.

 

What do clients need to do before the appointment on the day?
Clients are encouraged to bring a water bottle and any supporting documents that could help report writing. Support documents brought, such as GP letters and diagnosis summaries, will be photocopied on the day. Please also bring any reading glasses and communication devices (e.g. hearing aids) if they are required.

 

What do the clients need to do during the assessment?
The clients will answer the questions that are required for information gathering and report writing. They might be asked to complete the assessments and questionnaires.

 

Who will submit the report?
The report will be sent to the clients after two weeks. It is the client’s responsibility to submit the report when applying for NDIS. Further guidance on the application process will be provided after the assessment.

 

Could the client bring someone along?
Certainly! Clients are welcome to bring along someone significant to them. Their presence can offer valuable insights during the assessment process.

Are there any accessibility features in the building?
Yes, there are accessibility features in the building such as wheelchair accessible toilets.
For the Elizabeth location, we have a lift available to reach the second floor. Clients should check in at the reception first upon arrival. There are water fountains where clients can refill their water bottles.
